Packet request and receipt: The University of Colorado (CU) Polar Medicine administration receives packet requests. PQ packets (forms and instructions) are emailed to the recipients.
Packet completion: Participants complete required documentation, testing and examinations as identified in the original email.
Packet submission: Submission occurs via Epic Chart and My Health (CU Admin creates Epic Chart, Applicant creates MyHealth Connection Chart).
Records review and follow-up
Initial review: Once the packet is submitted, medical and dental records are reviewed by the CU Polar Medicine administration. Missing items are identified and requested.
Follow up: Additional documentation and/or missing items are requested by the administration.
Final Submission: All documentation is submitted. Administration reviews for completeness and forwards for medical and dental reviews.
Final review/adjudication
Physician and dental reviews: Packet reviewed by physician (medical and labs) and dentist (dental and X-rays). Identifies medical/dental concerns that may affect qualification based on the criteria in the PQ Guidelines. Additional information/documentation may also be requested during this step.
Initial determination: The participant receives either a Physical Qualification (PQ) or Not Physically Qualified (NPQ) notification in writing.
Waiver process: If NPQ, a waiver can be requested (directions are provided in the determination letter.) Once the required information is received, it will be forwarded to NSF for final review and determination.
